Description
Performs psychosocial assessment within 72 hours of admission.
Performs initial discharge planning assessment and makes appropriate referrals within the first 24 hours of contact.
Conducts all group therapy sessions as assigned. Documents all sessions and interventions in the medical record.
Conducts individualized and family therapy sessions. Documents all sessions and interventions in the medical record.
Qualifications
Masters Degree in Social Work from a college/university accredited by the Council of Social Work Education.
One (1) year social work experience in a clinical setting.
LCSW-I with completed hours minimum.
